We have an opportunity available for a Licensed Practical Nurse (LPN) to provide medical care for skilled nursing residents in a Long-Term Care Facility / Nursing Home in Verona. As a Licensed Practical Nurse (LPN), you will supervise nursing personnel to deliver nursing care and coordinate care delivery, which will meet patients' needs with professional standards of practice through physician orders, center policies and procedures, and federal, state, and local guidelines.

Responsibilities:

  • Provide compassionate, direct patient care under the Practical Nurse Act of Pennsylvania.  
  • Effectively communicate both verbally and in writing with all healthcare team members. 
  • Assist the healthcare team in managing treatment plans and health checks.
  • Administer medications and injections, take vital signs, perform basic diagnostic tests, and dress wounds per the health care provider’s orders according to federal, state, local, and facility policies and procedures. 
  • Observe, evaluate, and report changes in patient symptoms, condition, and reactions to medications or treatments. 
  • Assist and support staff by responding to patient call lights, answering telephones, passing meal tr, and transporting residents. 
  • Observe, evaluate, and report changes in patient condition. 
  • Communicate with patients and their families to thoroughly understand their treatment plans. 
  • Supervise and monitor patient care provided by unlicensed staff.
  • Support patient and staff culture of safety in the workplace.   
  • Recognize hazards and report promptly according to facility policies.

Qualifications:

  • You have strong clinical skills and can react decisively in emergencies.
  • You have impeccable verbal and written communication skills.
  • 1 year of nursing experience is preferred.

The Following Credentials Are Required:

  • Valid Licensed Practical Nurse (LPN) License
  • Valid CPR/BLS Certification
  • Current TB Testing Documentation
  • Current Physical Examination
